Services for Individuals



Case Management and Care Coordination: The average U.S. Medicare beneficiary sees between 5 and 11 different doctors each year, and the average Primary Care doctor is usually responsible for over 2,500 patients.  This can result in communication issues and gaps in care. Our Registered Nurse Case Managers conduct a comprehensive assessment, develop a care plan and help you or your family member know more about the condition, the treatment plan and better understand the specific team members responsible for each aspect of care.
 



Patient Advocacy: Our patient advocates are licensed professionals (Registered Nurses, Social Workers and Mental Health Counselors) who can help you or your family member navigate the often confusing and sometimes stressful health care maze.  We can support you, or with your permission can act on your behalf, working with the health care team to remove obstacles, identify resources and ensure that your overall treatment plan is tailored to your needs and requirements.  


 

Health Care Reimbursement Assistance: Medical bills are the leading cause of bankruptcy in the U.S., and it’s often hard to determine the financial impact of your care until after the services have been rendered. Our team knows who to call and the right questions to ask to make sure you get the most out of your insurance or other medical benefits. We help organize health insurance paperwork, review medical bills, contact providers or insurance companies to resolve claim issues, and track claims to ensure they are properly processed.  We can also help identify and link you with additional resources that may be available at low or no cost to you.
