Healthier Florida: A comprehensive disease management, care coordination and health literacy initiative aimed at improving the health of Medicaid beneficiaries in Florida chronically ill with Asthma, Diabetes, Hypertension or CHF. Partnering with five hospitals and over 50 care managers, we reduced access barriers and delivered scientifically sound interventions tailored to individual patient literacy, cultural and lifestyle needs. The project resulted in 34% improvement in self-care abilities, an 18% reduction in ED visits, and a 22% reduction in inpatient hospital days.





Kansas City Collaborative (KC2): Building on the established link between employee health and corporate bottom-line, this comprehensive health benefit program offering for large and mid-sized employers helps them prioritize and integrate disparate employee health data sources, and use that data to classify risks, align incentives, implement evidenced-based health interventions and assess the impact on employee health. The program increased employee participation in annual health risk assessments and biometric screenings by 17%, reduced BMI by over 7% and the average savings realized per employer was 17% greater than the implementation cost.







Transitions of Care: Supporting a professional association in bringing together a diverse group of health care stakeholders, the project delivers a set of standards that guide seamless transitions and support payer and provider organizations in achieving care coordination goals. This effort is designed to reduce costs and improve longitudinal outcomes for patients with complex care needs as they transition through various care delivery settings. The impact of implementing the transitional care standards is currently being evaluated using metrics established by CMS, Medicare STAR, HEDIS and CAHPS.
